ROCHESTER - The Rochester Fire Department has begun a File of Life card program.
The File of Life card enables emergency responders to obtain a quick medical history during a medical emergency.
The File of Life card lists the patient name, emergency medical contacts, medications and dosages, surgeries, medical conditions, allergies, insurance information and healthcare wishes.
Anyone who wishes can join in the program using the directions below.
1) Print out the File of Life card
a. Click the link to access the FOL Card: https://www.rochesternh.gov/fire.../files/file-of-life
b. If possible select print on both sides
2) Complete all relevant information.
a. Hand written or by using the fillable PDF document
3) Hang the card on the refrigerator
a. To protect your medical information from being visible the department recommends residents fold the card in half and place it in a clear plastic sandwich bag
